Holding Steady

Things are holding steady here.  Our VMA/HVA numbers were almost the exact same last week as the week before.   We’d like to see them go down but happy they are not going up.   We had a great time with my dad and Stephanie in town.  They were great with the boys and a huge help.  Grandpa went to Sam’s school and gave an astronomy talk about the sun, moon, planets and the solar system. He was a huge hit with the kids and Sam’s teacher, Mrs. Abuhl, elbowed me during his talk and said “Can he come back next week and talk about stars?”  She was bummed to learn he lives in New Mexico.  Sam has been working hard on his homework after a week and half of knock down drag out homework fights during which, to put it nicely, I was not at my best.  I think we’ve got a system down but we’ll see how it works out.  Neil’s been working really well with him on flash cards. We just gave the kids back their video games after five days without them so we’ll see how it goes.   Andy is doing fine.  He went with us to the hosptial  yesterday for Sam’s blood check to keep him company and the boys passed the football in the halls.  The boys were teasing Charlie to the point of tears after we got home and I told them Santa was watching (Charlie had just watched Rudolph).  Andy got very concerned and when I came down the stairs later he asked, “Mom, can you play with coal?”  I know, it’s not even December and I’m resorting to Santa - really working on calm unflappable patience in leui of shrieking, Santa threatening mother. 

Soccer season is winding down and basketball starts soon. Charlie is starting “arms” in swimming lessons and playing with his firetruck and Clifford lately.  We put out fires all over the house.

Next week is a big week for us.  Both boys have their parent teacher conferences (Andy’s is Tuesday and Sam’s is Wednesday).  Those will be put immediately into perspective because they are followed by Sam’s scans.  Wednesday is his injection and Thursday is a bone marrow biopsy to be sent to Dr. Sholler followed by an MIBG scan.   

I’ll try to post this weeks numbers as soon as I get them.  We’ve been getting them back earlier since we started doing checks on Monday vs. Tuesday.

The boys had a great Halloween starting with their Halloween carnival at school where they played a lot of games and Andy got his hair sprayed.  Sam dunked Mrs. Emerson in the dunk tank (thanks for being such a good sport Mrs. Emerson) and they both played all of the booth games.  Charlie was home sleeping with my dad and Stephanie.  He is having a great time this week with both of them as are all of the boys.   The Monacos came over Halloween night for pizza and then we went trick or treating around our neighborhood.   The picture above of Andy and “most excellent” Andrew was because this guy we saw on the street – a real Spicolli- was so happy when he saw Andrew’s costume, “Dude! are you Tom Brady?  Excellent!  Guys, this little dude is Tom Brady!  That makes my night.  I’m a huge Patriots fan! Dude.  Excellent!  What’s your name?”  Andrew said “Andrew”  and the guy said “No Way, my name is Andrew too!”  He was very funny and so happy to see a Patriots fan.  I think he told Neil he was from New England but he had the surfer dude accent to the “T.”  So we took a picture of he and Andrew.  Andrew was very happy to have created such enthusiasm.  Later he said “Mom, this was the best Halloween ever. It would have been equal with all of the other Halloweens because I love Halloween, but this one was better because I got to take a picture with that guy.”  It’s funny what strikes kids. Sam loves Halloween too.  It’s his favorite holiday and he was happy and energetic all day.  He was so excited to get to trick or treating he kept saying, “Now? Now can we go?”  Also, as usual, he was very die-hard about it.  Even after we hit all the houses and were on our way back he kept seeing houses we might like to try.  Charlie caught right on to the trick-or-treating thing and as soon as he discovered there was candy coming, our shy little boy was right in there saying “trick-or-treat” and “thank you.”   Grandpa carried his pumpkin for him because it was too heavy so he’d run back and put his stuff in after every house.

The boys both did very well at soccer this weekend.  Andy scored seven and the other team didn’t score any.  It was a complete rout.  Sam’s team lost but it was a very exciting game and close.   Neil helped coach because the assistant coach wasn’t there and he had a great time, he was practically more excited than the kids.  Sam did really well in the second half and went after the ball a lot and dribbled it down field a couple of times.   My Dad got to see both games and cheered everyone on.  Stephanie saw Andy’s game and cheered him and was home getting ready for dinner during Sam’s game.  Yesterday was pretty quiet the boys went over to the place where my dad and Stephanie are staying in the afternoon to play board games.  My dad was funny after playing Sorry with Sam and Andy.  He said amazed “Sam didn’t exactly follow the rules.  If he didn’t like a card, he’d put it back and take mine.”   If you’ve ever played a board game with Sam, he has some creative ways of making himself win -some might even say sometimes he cheats, usually when he’s playing with someone who’ll let him get away with it.  Charlie fell asleep and Neil and I had a few hours when the house was quiet.  We kept marveling at the quiet.

Charlie and I dropped off Sam’s urine this morning after dropping the boys off to school.  Last week’s VMA/HVA was: VMA 11.8 (up – expletive here), HVA 12.7 (down, yay).   So we’ll see what this week brings.  Dr. Willert is setting up a bone marrow biopsy and an MIBG scan for us (we’re due).   Sam has been happy and enjoying himself.  We’re really working on his math and reading.  He’s having a good time with my Dad and Stephanie in town.  I may tackle them before they get on the plane on Friday.

When is Your “Not Too Distant Future?”

I know that I am being a scary complainy and bitter person and I promise to stop but I just thought I’d post the following Google alert I received today:

Researchers develop novel anti-tumour vaccine Science Centric | 30 October 2008 14:12 GMT

A novel anti-tumour vaccine for neuroblastoma and melanoma developed by scientists and clinicians at Children’s National Medical Centre in collaboration with investigators from the University of Iowa is showing significant impact on tumour growth in mice, according to new research published in the October edition of the research journal Cancer Immunology, Immunotherapy. The vaccine uses the tumour’s own protein to induce an immune system response, allowing for a personalised approach to treatment.

The vaccine and delivery system, developed in the laboratory of Children’s National Chief of General and Thoracic Surgery Anthony Sandler, MD, involves the creation of synthetic microparticles known as ‘immune stimulatory antigen loaded particles’ (ISAPs), that consist of tumour antigens (proteins) from the specific tumour to be targeted, as well as immune stimulatory agents. The ISAPs are detected and engulfed by specialised immune cells and sensed to be immune-stimulating ‘foreign bodies.’

The study shows that ISAPs are effective at blocking the growth of tumours in mice by inducing activation of immune cells that then stimulate the immune system to specifically target the tumour whose antigens match those that are loaded in the particles – known as tumour specific immunity.

The research team also discovered, however, that the impact of ISAPs on tumour growth was partially mitigated by an increased presence of regulatory t-cells (T-reg) when ISAPs are introduced into the body. The researchers believe that T-regs play a key role in how the vaccine impacts tumour growth by suppressing the development of the specific immune cells needed to combat the tumour. By adding a T-reg suppressor such as cyclosphosphamide or anti-CD25 antibody, the scientists were able to have a greater impact on preventing tumour growth using the ISAP approach.

‘For tumours like neuroblastoma, reduction to minimal residual disease with standard therapies like chemotherapy and/or surgical resection and subsequent treatment with this vaccine could quite possibly cure the patient of the disease in the not too distant future,’ said Dr Sandler, lead author of the study. ‘Creation of ISAPs allows us to target our treatments to the specific tumour of interest, a capability that will more effectively combat a wide range of these tumours in a personalised fashion.’

So my “not too distant future”  is in two hours when I go to the elementary school and help two very excited boys into their Halloween costumes.  I’m guessing though, that is not the kind of “not so distant future” they are talking about in this article.   I do know the realities of research and all and I really am not anti-research people.  I just want it to go faster, I feel like we are in race and if, God forbid, we are ever in the position where we “just missed it”  I think I will go crazy.

I’ll have Halloween pics up later this weekend.  The boys are as cute as can be.  We carved our pumpkins last night and are very geared up for trick or treating.  Sam and Andy have been practising with Charlie so he knows what to say.  Ok I’ll stop being bitter lady and enjoy our Halloween.  Sam is feeling great, you should have seen him in soccer practice yesterday, he scored two goals and had fire in his eyes.

Halloween/Halloweden

First of all, I have been very remiss in not reporting on our website about HallowEden and event that the Brunskows are put together for Halloween to honor their beautiful Eden.  Volunteers dyed their hair orange (Eden’s favorite color) for Halloween to raise money for MagicWater.  Here is a link to the story on the front page of yesterday’s Orange County Register: http://www.ocregister.com/articles/eden-cancers-event-2203872-halloween-sunday   They’ve raised close to $30,000.   Awesome.

The boys are counting down to Halloween and are very excited for their school carnival on Friday and Trick or Treating.  Sam woke up this morning and said “Only three more days until Halloween.”   My dad and Stephanie are coming in to town tomorrow and will be here for all of the festivities.  We can’t wait to see them. Yesterday after I told Sam that Grandpa would love to help him with his night sky log for his homework, he’s learning about the moon, the sun, the planets, etc. and he loves it.  He keeps filling me in on all the facts he’s learning.    Anyway, I told him Grandpa is a scientist and knows a lot about space. Andrew said, “What is this Grandpa, Grandpa Grandpa?  Does, he know everything? Yesterday you told us he was a good at yo yos” (the boys bought a yo yo when we were out the other day to get ref. shirts for Neil and me).   Sam also told me yesterday, “Mom, I like reading and writing now.”  Be still, this readaholic mother’s heart and thank you vision therapy.

Charlie and I have been going for a run in the mornings after we drop the boys off at school.  When I took the other two boys in the jogging stroller when they were little, they were always letting me know they were ready to get out five minutes into the run, but Charlie is so mellow. We comment on all of the birds and trash trucks we see and then when we get to the beach we sit on a bench and watch the surfers and Charlie waves at the birds and says, “Hi birds. Oh, they didn’t wave at me.”   It’s been beautiful weather here in the early morning – though hot in the afternoons.  I figure we might as well make something good come of the fact that the boys have to be at school at the insane hour of 7:35.  Anyway, running helps clear my head and stave off panic attacks regarding Sam’s numbers.   He and I did a blood check yesterday after school and Andrew had a playdate. We threw the football in the clinic while we waited.   His numbers are below.   We know HVA/VMA might go up because we hadn’t taken him off Etoposide yesterday.  At least VMA didn’t go up too much:

HVA = 15.1 (expletive here)

VMA = 9.5

WB: 5.5

HM= 11.8

ANC= 2,940

PLT = 282 

LDH = 788

AST=48

ALT= 23

ALK/PHOS (following for his ankles)= 187 (on the low normal range).

Neil and Pat Lacey are in New York meeting with, Meryl,  Dr. Sholler and some docs from MSKCC working on MagicWater stuff.   Neil gets home tomorrow.   There are some exciting things happening on the research front and I just want it to go faster.  I yell at my computer screen (probably the way some people are yelling at their TV screens about politics these days) over NB Google alerts, about some new promising treatment and then in the summary always the CYA statement that whatever it is still has to go through much more testing before it will get into actual people.  Thank goodness for Dr. Sholler and her work and her sense of urgency.

The Dark Side of the Moon

Sam very earnestly told me the other night when we were out for our after-dinner family walk and it was a full moon, “Mom, you know  you aren’t really seeing all of the moon.  That side is the side lit up by the sun but there is a whole other side that is dark because it isn’t being lit up by the sun.  Sometimes the posts on this website are only the bright side of the moon so to speak which is funny because people have told me they can’t read it because it’s so depressing.  I do post the dark side sometimes because it is our reality. 

Our bright side of the moon this week:

Sydney Dudley had clean scans!  Always love to read about clean scans and Sydney has a special place in our heart. Go Sydney!

 Sam and Andy both had great soccer games over the weekend.  Sam was awesome on defense, especially in the first half and seemed to have his confidence back after last week.  Andy scored and scored and passed a  lot this week too so his teammates could score.  I think Neil got a photo of him doing his dance.  I’ll post it if we got it.  We had a wonderful night at the beach Friday night with the boys just playing tag and running in the sand. Watching them run from Neil and laugh silhouetted against the sunset was magic. We met the Bonebrakes at the park on Sunday afternoon.  It was great for the boys to get together since they haven’t seen much of each other since school started.  They played football and kickball and argued quite a bit but had fun.  Kurt was a wonderful referee. 

Charlie is funny as can be.  He’s doing a MyGym class on Fridays which he keeps calling “YourGym” because I call it “MyGym.” He also has swimming lessons so we can get him water safe.   As much of a ham and a clown as he is at home, he is a complete stone when I take him out in public.   He’s just barely warming up to his swimming teacher after 13 lessons and at MyGym all he wants to do is jump in the ball pit (I always think of that Will and Grace episode when Karen yells at Grace to get out of the ball  pit  because it’s “a pit full of pink eye”) when it’s circle time he keeps his hands rigidly at his side and is a complete stone face to the point where it is actually funny.   His two funniest tricks are picking up the handset on my fax machine and talking  into it: “Hello Elmo? What are you doing?  Oh, you went a the zoo?  I went a the zoo too.  Mmm hmm mmm hmm.  Bye.”   He also does this awesome Darth Vader with the light saber when he rolls his eyes up in the top of his head and makes his voice very dark and deep and says “You come a Dark Side.”  

Sam and Andy read together on the couch today so beautifully.  They both have to read for a half an hour a day and I’ve found Sam loves reading to Andy and then Andy  reads his little books to Sam and Sam helps him with the words.  I think it’s good for Sam’s confidence and it wasn’t a struggle to get them to read for once.  It was funny to listen to them.  Sam was asking Andy which of the books he wanted to read and he remembers them from when he was learning,  he said, “Ok these are the funny ones so you should pick from these three.  This one is the funniest so you should pick this one.”  I was thinking of Andy channeling Kramer, “Why don’t you just tell me which book you’d like me to pick.”   Andy has a playdate tomorrow with Beau and he is so excited he can’t stand it.  He’s been counting down since I told him about it.  I volunteered for a little while in his classroom today and he just beamed when I walked in.  He’s doing really well in school and loving it.  Sam has a spend the night party for Tanner’s birthday on Friday and I’m working on figuring out getting all his meds in him so he can spend the night.

On the dark side of the moon:

I had a work week last week that made me wish that I was living on the Island of the Blue Dolphins, just me and my loyal dog Rontu, collecting abelone and digging roots for survival. I was really struggling for perspective.   Picking Sammy up from school today and seeing a 55% on  his math test made me think perhaps I’d better take the whole family to the Island and we can all become abelone collectors and root diggers.   Sam’s VMA/HVA numbers came back late this afternoon and his VMA jumped up to 9.5, HVA 11.6.  Nothing like life coming along to kick us in the teeth to give me the perpective I was so desperately seeking last week.  We had switched chemos from oral temodar to oral etoposide and now we think we will switch back.   We dropped off urine at the hospital this morning so next week’s numbers may be up as well since the results from today will still be with etoposide.  We are trying not to panic as they are not crazy numbers but whenever numbers go in the wrong direction it’s impossible not to get that sick feeling and freak out.   

Sam turned his ankle pretty hard at school today and had to go to the nurse’s office. He’s been turning them more than I’d like to see lately so we need to keep a close watch on his ankles.   We had an appointment last week to reprogram his hearing aides as he’s lost more high frequency hearing in right ear. Our audioligist, who we love, was amazed at how good Sam is at gleaning information from context in sentences and I was floored at how hard he was working to figure out how to repeat her sentence back to her so that it made sense.  This was with his hearing aides on. He obviously missed a lot of the words but he’d take what he did hear and  try to make sense of it.  It almost made me cry to think of him doing this in school.  Thank God for the sound system.  I hope with having reprogrammed his hearing aides, he’ll have an easier time of it.   I hate cancer and I hate the stupid treatments.  I hate it.  Hate it. Hate it.

Little Hector Soto died last week.  Neil went to his funeral on Sunday and sat next to his devastated father and family.  Please think good thoughts and say some prayers for them.

Thirteen… PLUS 6.6

Just adding a number to Neil’s post below.  Sam’s VMA is 6.6! NORMAL, NORMAL, NORMAL!  I’ll  post a Sam update tomorrow after blood check. He had a rough soccer game on Saturday and has lost more of his hearing per his audioligy exam last week-sucks - but he had a good day at school today and is in good spirits.   His VMA numbers are awesome. Wohoo! Neil’s post  on Andy below…..

The beauty of having a blog is that we’ll be able to look back 20 years from now when all 3 boys have wonderful girlfriends and solid educations, and relive some of the great times we are having.  Saturday was one of them.  Usually “soccer Saturday” is stressful as juggling 2 kids while watching the 3rd child play incurs more juggling and less watching.  Sam and Andy had overlapping games so Andy chose me to chaperone his game 🙂 I vowed “no cameras” since I am often so caught in documenting moments, that I miss the moments.

 All I can say is that my budding Pele scored 13 goals — each followed by a distinctive dance that made me and the crowd laugh.  My favorite celebration was Michael-Jordan-channelling-Charo dance where Andy’s tongue is sticking out & curled, hands are on hips, and he’s dancing a frantic two-step.  8 goals in the first half; battlefield promotion to a different field followed by 5 more.  Andy truly loves running and I actually saw him pass the ball with his right foot (he’s so strongly left-footed that this is a true shock).  So, Andy — when we read this together 20 years from now, I just want you to know how much you made me laugh & smile on Saturday.

Once Every Two Weeks

We are moving our blood checks from once a week after school to once every two weeks.  Since we are currently on low dose temodar with some other things mixed in, Sam’s ANC, Red blood and Platelets have been fine. His VMA last week was 7.8!!! and HVA 10.1.   We are keeping weekly VMA/HVA checks.  Charlie and I just dropped off Sam’s urine after we dropped the boys off to school.  Charlie likes to comment on all of the trucks along the way to the hospital and then I let him drive out of the parking lot and give the lady the parking ticket and then pop him in his seat. 

The boys are gearing up for Halloween.  They are all three going to be football players. I tried to get Charlie to be Thomas the Tank Engine and even bought him the costume but he was having none of it.  I had to take Thomas back.  Sam and Charlie are going to be LaDanian Tomlinson and Andy is going to be Tom Brady.  I was trying to get him to be Tom on crutches but he said “no way.”  He’s Tom Brady before he blew out his knee.  The boys and I went out to dinner last night after we went looking for shoulder pads for their costumes and the Andy was wearing his Tom Brady jersey and the waiter kept referring to him as “Mr. Brady.”  Andy loved it. “Can I get you anything else Mr. Brady?” It was funny.  Neil and I are going to be referees which is a pretty good reflection of life as we know it.

Neil is in Vermont meeting with Pat Lacey and Dr. Sholler and was in Philly yesterday for the pediatric cancer foundation seminar set up by the amazing Liz and Jay Scott, parents of Alex Scott and founders of Alex’s Lemonade Stand. I’ll report on theat when he gets back.  

Sam has been feeling good.  I go to school twice a day to give him meds and he is always engaged and happy.  They have set up such a great sound system for him in his classroom.   Diane Levy, our hearing specialist is so so good. She really advocates for Sam and she “gets him” and likes him - which I love.   I don’t tell her how much we appreciate her enough, so Diane, if you are reading this, we love you!  Sam’s homework and schoolwork coming home looks really good.  100%s, Five out of fives, etc. and his writing and reading are really coming along.   I think vision therapy has made a huge difference.  Tomorrow is back to school night it’ll be the first time we have two classrooms to visit so Neil and I will switch off.  Sam and Andy and I have been reading Hank the Cowdog books before bed and we’ve been laughing at Hank and Drover (the two main dogs in the story).  Sam thinks Drover is so funny.  In the last book we read Hank fell in love with a Collie dog and I started teasing the boys when I tucked them in saying “Go to sleep and dream about girls.”  They laugh so hard every time I say it thinking it’s the funniest thing ever.  “As if, Mom.”   I know some day it’ll be true but it’s fun to tease them about it now.
I know this is sort of a “la la la” ostrich type update but things are good and I’ll take it.  We’ve had good numbers and Sam has been having a normal boy life.  His team won in soccer last week and he was so happy. He played goalie for a while and did great. His coach told him he had a “shut out”  which was very cute because though Sam ran and got the ball when it was in his vicinity, there weren’t any actual shots on the goal.

Update at Long Last

We are all fine.  Sorry for the long delay on updates. Sam’s VMA/HVA numbers came back today VMA: 8.8 (pretty good) and HVA 11.9 (little high).  VMA number made us very happy today. 

The boys have been busy with school, soccer, being fanatic football fans and life in general.  The DeLongs had extra Padre tickets this past Saturday and gave us some great seats.  The boys all brought their gloves and wore their Padre shirts.  Yesterday they wanted to go to the park and play catch and Charlie for the past two days has carried a baseball with him everywhere he goes.  We are a sports crazy house these days.  

Sam is doing well in school and soccer. He almost scored a goal this past weekend.   There hasn’t been any drama around here lately aside from the normal everyday life squabbles between Sam, Andy and sometimes Charlie- though I did set a pan on fire trying to broil fish while Neil was at the hospital last night and scared the bejayzus of Sam, Andy and myself.  Sam and Andy reinacted it several times and Neil all about it when he came home.  Who knows what the third grade and kindergarten will hear today at school. Charlie is in swimming lessons twice a week but now on his second  set of lessons still won’t put his face in the water.   He enjoys himself at lessons though as he does at most things.

I promise to update more frequently.  Yikes on the stock market today.  Funny how numbers can make the world of difference in an outlook.  Happily our most important numbers looked good today.  Hope everyone else can find a number to be happy about too.

Football Season

It was a beautiful night last night and we took the football up to the park.  The boys had a great time as you can see.  We had the park to ourselves and it was just one of those unexpected times I wanted to soak up the moment - simple as it was.

We had our radiation consult this morning and Dr. Mundt thought we should just consider it the start of a discussion of whether or not we should radiate and if we do, if we do something different in terms of dosage or timing.  The spot on the femur is the exact same spot we radiated in 2006 at relapse (exact same).  He said it isn’t common for the spot to go away and then reappear in the exact same place.  So, you have to assume that radiation plus chemo worked on most of the cells but there must of been some cancer cells that got away and here they are back again merrily reproducing.   Does it make sense to go back and re-radiate?  Don’t know.  He wants to talk to some radiation specialists at the COG meeting in mid-October and make a decision then, since we have been improving and it isn’t all that time sensitive that we start radiating if that is what we are going to do.  So, if we do decide to do it, we probably need a new play, since the old play has been seen before (had to go with a football pun given the title of this post).  

We have a blood check after school today and if the numbers stay in the right range, we may be happy to just think about radiation for the time being.

Andy and Sam are still really enjoying school, though Friday of last week Andy woke up and said, “I have school again?”  Five full days is a long time for him.  He’s made some new friends and Sarah is his favorite girl in class.  Charlie was bothering him yesterday and he said, “Charlie, your nerves are getting on me.”  It was funny. Sam got back from radiation in time for snack recess.  His writing and reading teacher from last year tested him on Friday and said he’s made leaps and bounds over the summer (vision therapy I think).   Yesterday Mrs. Abbuhl asked the kids to write about someone who was special to them and Sam wrote about Charlie.  I haven’t seen it yet but his writing teacher told me about it this morning.   I’ll post it if I can get my hands on it.   He’s getting a group of buddies in his class and really likes Mrs. Abbuhl. 

Sam’s team lost their first two soccer games.  He’s been playing defense mostly and is a little intimidated by the bigger boys coming at him but he mixed it up a little more this last weekend.  Andy’s team is running roughshod over the competition and Andy’s been scoring a lot.  Now if we can teach him some humility…

Despite all the good news, we’ve been pretty stressed out and terrified lately.  Last night I tried to make myself  stop obsessing about everything and enjoy the beautiful night we were having.  I seem to be in a constant state of worry lately.  I hadn’t realized how little sleep I was getting until I slept through the night on Friday night.  I woke up and it was still dark and I figured it was 1 or 2 in the morning (my usual) and I thought I’d call my brother in Australia and then I heard the sprinklers going and realized it was 6am.  Wohoo.

Please think good thoughts and say some prayers for Hector Soto and his family. He doesn’t have a website but he is being treated for NB here in San Diego and is not doing well and in a lot of pain.
